Identification du patientAu moment de l’admission du patient en hospitalisation stationnaire ou de jour dans un hôpital, les professionnels de santé ou le personnel administratif d’accueil assurent, selon les procédures arrêtées par l’hôpital, un identifiant permanent pour ce dernier.Lorsqu’il ressort de l’identification du patient que ce dernier dispose déjà d’un identifiant permanent et d’un dossier archivé, ce dossier est rouvert. Dans tous les autres cas, un nouveau dossier est ouvert et un identifiant permanent est attribué au patient.

Contenu du résumé clinique de sortieLe résumé clinique de sortie comporte au moins les éléments décrits dans l’annexe 3.En cas de décès du patient, le résumé clinique de sortie comporte au moins les éléments 1, 2 et 3 du volet A, les éléments 1 et 2 du volet B et les éléments 1 et 2 du volet C de l’annexe 3 et mentionne les circonstances, l’heure et les causes suspectées du décès. Il précise si le patient a bénéficié d’une prise en charge palliative. L’identité et la qualification du médecin qui a constaté le décès sont consignées.
